The Referral Services Center at UMass Memorial Medical Center offers
community physicians the following services.
For outpatients:
- Assistance in arranging outpatient and physician appointments at UMass
Memorial Medical Center
- Information about specialized services offered at UMass Memorial Medical
Center
- Medical record, lab and other test results
- Coordination of patient information necessary to request an appointment
For inpatients:
The following inpatient information is sent to each
patient's primary care and referring physician:
- Admission letter, including admission date, diagnosis and name of UMass
Memorial attending physician
- Discharge order form identifying the clinical events and medications for
patient at time of discharge
- Discharge summary for patients with a hospital stay beyond two days mailed
upon receipt from Medical Records

office is staffed weekdays from 8 am to 5 pm.
