The Home Health Program at UMass Memorial Medical Center in Worcester assists clients in remaining independent and provides them with quality health care services at home. We have served Worcester County residents since 1966, providing skilled nursing, therapy and health education programs to people of all ages.
Our home health staff believes in preventing hospitalization by delivering health care assistance before a problem becomes a crisis. In addition, we offer an opportunity for early hospital discharge.
An individual care and treatment plan is developed for all patients receiving home health services through collaboration between the patient and the home health nurse, the patient's physician and family members. Services may include:
- Skilled nursing care (e.g., health assessments and symptom management)
- Physical/occupational therapy
- Speech therapy
- Medical social work (e.g., client and family counseling and financial planning)
- Home health aide (e.g., bathing/personal care, feeding/meal preparation, walking)
UMass Memorial Home Health is licensed by the Massachusetts Department of Public Health and certified by the Joint Commission. Our service ranks us in the top 25 percent of home health providers nationally for quality performance in outcomes and improvement by HomeCare Elite in each of the past four years. In fact, we are the only home health service in Worcester County to achieve this recognition!
